The hosting infrastructure processes technical request information to deliver and protect the Website. Contact submissions are sent directly from your browser to Web3Forms, which processes technical request information for delivery and spam prevention. The separate recruitment endpoint, when enabled, uses a hashed IP-derived identifier in temporary memory to limit submission attempts. These controls do not create a visitor account.

Attachments and Form Delivery
The Contact form uses Web3Forms to process your contact details, message, and any optional attachment and forward them to the mailbox configured for Apex Radiology. Web3Forms and the receiving email provider may retain submissions and attachments according to their policies and account settings. Contact attachments are limited to one PDF, JPG, or PNG file up to 5 MB. Do not upload patient information, medical records, or sensitive personal documents.
The separate careers form, when enabled, uses Resend to send expressions of career interest to the relevant Apex Radiology mailbox. CVs and résumés are processed in application memory and sent as email attachments. The application does not save contact or recruitment attachments in a public uploads folder or publish them on the Website.
The email provider and receiving mailbox process and retain messages and attachments. A successful form response indicates acceptance for email delivery, not confirmation that a team member has read the message. If forms are unavailable, you may contact us by email, subject to the same restrictions on patient and sensitive information.
Information Sharing
We may share information only:
- With hosting, form processing (including Web3Forms), email delivery, and mailbox providers as needed to operate the Website and process inquiries
- With the Apex Radiology personnel responsible for your inquiry or recruitment interest
